What to Bring to Your First Primary Care Appointment

Thank you for booking an appointment with Passion Health Advanced Primary Care. We are excited to support you on your health journey. Here are a few simple tips to help you make the most of your first visit.

Confirm Your Personal Information and Account Details

Before your appointment, please make sure your personal details are complete and up to date. 

This may include your full name, date of birth, phone number, email address, home address, insurance information, and preferred pharmacy.

You can also log in to the Passion Health Patient Portal to review your health record, manage appointments, communicate with your provider, view test results, request refills, and pay bills securely online.

What to Have Handy

It is helpful to keep the following items ready for your visit, as your provider may ask for them:

  • Previous medical records

  • Recent lab results or imaging reports

  • Notes from your most recent annual physical or wellness exam

  • A complete list of medications, including dosage and instructions

  • A list of allergies, including medication or food allergies

  • Names and contact details of your previous doctors or clinics

  • Insurance card and photo ID

If you already have medical records, lab results, medication lists, insurance details, or previous doctor notes, you can upload them before your appointment through the Passion Health Primary Care Patient Portal. 

Go to the Passion Health Primary Care website > Click on Patient Resources page > Patient Portal 

  1.  Log in to Patient Portal and access your health record. From there, you can share important health documents with our care team before your visit. 

  2.  You can also use the portal to view test results, manage appointments, communicate with your provider, request refills, and pay bills securely online.

  3.  This helps your provider prepare for your appointment and makes your first visit smoother, faster, and more personalized.

  4. This helps our care team review your health information ahead of time and prepare better for your first primary care appointment. 

Pre-Visit Forms or Questions

Depending on the type of appointment you booked, you may need to complete a few forms or answer basic health questions before your visit.

These details help your provider understand your symptoms, health concerns, medications, lifestyle, and care goals before the appointment starts. Completing them early can save time and make your visit smoother.
