  2. Start by selecting your preferred clinic location from the appointment page.

  3. After choosing a location, available providers and doctors for that clinic will appear below.

  4. Once you select a provider, you will be directed to the appointment form. 

  5. Select whether you are a New Patient or an Existing Patient 

  6. Tell us how you heard about Passion Health Advanced Primary Care 

  7. Choose the reason for your visit.

  8. Enter your personal and insurance details 

  9. Submit the Appointment Form 

After submitting the form, you will receive confirmation for your upcoming appointment.
