Short answer

Can I get prediabetes care online in Texas?

Yes, for adults 18 and older in Texas. Borderline A1c? A clinician reviews your labs and risk and helps you plan steps to lower your chance of type 2 diabetes. Review is by a Texas-licensed Passion Health clinician, 24/7 on weekdays and 7 AM–9 PM CT on weekends. The flat price is $89, or use insurance. If labs are needed, your clinician usually orders Hemoglobin A1c, $41 including the $29 draw-and-review fee, through Passion Health Blood Work, drawn at a Passion Health clinic or any CPL patient service center in Texas; with insurance, the lab bills your plan directly. A prescription goes to the pharmacy you choose only if it's appropriate; controlled medicines are never prescribed online. If you have very thirsty and urinating much more than usual, with weight loss you can't explain, you need to be seen in person today instead (at our Dallas–Fort Worth clinics, or urgent care near you), and you don't pay the online fee. Already a Passion Health patient with insurance? We book you with your own clinician instead.

About the condition

What to know about prediabetes

Prediabetes means your blood sugar is higher than normal but not high enough to be type 2 diabetes. It is usually found with an A1c of 5.7% to 6.4% or a fasting glucose of 100 to 125 mg/dL. Most people have no symptoms. Risk rises with extra weight, inactivity, family history, PCOS and age.

What is insulin resistance?

Insulin resistance means your muscles, fat and liver don't respond well to insulin, so your body has to make more of it to keep blood sugar in range. Over time this can lead to prediabetes and type 2 diabetes. Signs that can go with it include dark, velvety skin patches on the neck or underarms, PCOS symptoms such as irregular periods, and snoring or poor sleep.

Before you start

Is online care right for you?

A good fit for online care

  • A recent A1c or fasting glucose was in the prediabetes range and you want it explained
  • You have signs of insulin resistance, such as dark velvety skin patches or PCOS, or a strong family history of diabetes
  • You want a plan for food, activity, sleep and weight, and to know whether medicine makes sense
  • You need a repeat A1c to see where you stand

Book an in-person visit instead if

  • Very thirsty and urinating much more than usual, with weight loss you can't explain Same day
  • You're pregnant and have been told your blood sugar or A1c is high Within days
  • A lump in your neck that is growing quickly Same day

Not near one of our clinics? Online Care is open to adults anywhere in Texas; our clinics are in Dallas–Fort Worth. If the safety questions or your clinician show you need to be examined, we tell you what kind of care you need and how soon — an emergency room, an urgent care center, or a clinician near you. If you've already paid, the full fee is refunded and you get a written summary of your visit to take with you.

Call 911 or go to the ER now if you have

Call 911

  • Drowsy or confused along with extreme thirst or frequent urination
  • Blood sugar under 70 that won't come up with treatment
  • A known adrenal problem with vomiting, very low blood pressure or confusion

Emergency room now

  • Vomiting, belly pain, or fast breathing along with extreme thirst
  • Blood sugar over 300 with ketones, vomiting, belly pain or fast breathing
  • A known thyroid problem with fever, racing heart and agitation

Also call 911 for chest pain, trouble breathing, stroke signs, a sudden severe headache, fainting, a seizure, severe bleeding, new confusion, throat swelling, an overdose or thoughts of harming someone else — and call or text 988 for thoughts of harming yourself. If screening routes you before you pay, nothing is charged. If a clinician routes you after you've paid, the full fee is refunded.

Already a Passion Health patient?

If Passion Health is your primary care practice and you have insurance, book this with your own clinician — in person or by video, billed to your plan like any visit, with no online fee. Your clinician knows your history and can adjust your whole plan, not just this one problem. Online prediabetes care is for new patients, uninsured patients and patients outside Dallas–Fort Worth; established patients without insurance pay the $39 renewal rate.

How it works

How your prediabetes visit works

  1. 1
    Answer the prediabetes intake

Upload prior results or home readings if you have them.

  2. 2
A clinician reviews it the same day

If labs would help, the order goes to a Passion Health clinic, or to any CPL patient service center in Texas.

  3. 3
    Results go to your clinician

Your clinician reviews them and contacts you with your plan.

  4. 4
    Follow-up on a schedule

Check-ins and repeat labs timed to your condition; 30 days of messaging.

Your care

What to expect from your clinician

What your clinician may consider

  • Reviewing your A1c or fasting glucose history, weight, blood pressure and family history
  • A repeat A1c, and sometimes a fasting glucose or cholesterol panel, if you don't have recent results
  • A plan built around food, activity, sleep and weight, with a dietitian referral if helpful
  • A medicine to lower diabetes risk for some people at higher risk, if appropriate
  • An evaluation for WeightCare if your BMI meets program criteria

Shown so you know what to expect. The reviewing clinician decides what, if anything, is appropriate for you — a prescription is never guaranteed, and controlled medicines are never prescribed online.

Labs, tests and Passion Health services

  • If your clinician recommends an A1c, you can have it drawn at a Passion Health clinic or at any CPL patient service center in Texas.
  • If your clinician recommends nutrition counseling, you can meet with a Passion Health dietitian.
  • If your BMI meets program criteria and your clinician recommends it, you can join Passion Health WeightCare, our medical weight-management program.
Price & insurance

How much does an online prediabetes visit cost?

Standard evaluation
$89
  • Everything in Quick
  • Lab orders when needed (labs billed separately)
  • 30 days of secure messaging

Using insurance: we're in network with BCBS of Texas, Aetna, UnitedHealthcare, Cigna, Humana and Healthcare Highways, and we verify eligibility for every plan before review — you see your estimated cost-share first. New patients have a short video visit, because plans pay for a visit, not a form. Same-day review applies to self-pay visits; insurance visits begin once we've verified your plan, which is the next business morning if you submit overnight or on a weekend. Medicare, Medicaid, CHIP and TRICARE are billed to the program, and the cash rate isn't offered for covered services.

Full refund if your case isn't accepted: if the clinician doesn't accept your case for online care, for any reason, the full fee is refunded to your card within 10 business days, and we help you get the care you need: a same-day visit at one of our Dallas–Fort Worth clinics or, if you're not near one, clear advice on where to be seen and a written summary of your visit. With insurance, no claim is submitted for the online review. Paying the flat price yourself? You can ask for a written Good Faith Estimate before your visit.

Labs your clinician orders are priced through Passion Health Blood Work if you self-pay, or billed to your plan directly by the lab (CPL) if you use insurance; imaging is billed by the imaging center and medication is paid at your pharmacy. Passion Health doesn't sell, ship or mark up medication.

Follow-up

A 48-hour check-in, 30 days of secure messaging, and lab follow-up when labs were ordered.

Expect a repeat A1c at least once a year, and sometimes sooner after a change in your plan. You can message your clinician with food or activity questions during your messaging window.

Labs your clinician may order

Priced through Passion Health Blood Work

A1c confirms the range; the Diabetes & Kidney panel adds cholesterol and kidney checks. Ordered only if your clinician decides it's needed — the visit price never includes a test you don't need.

All Blood Work tests
Hemoglobin A1cThree-month blood sugar average$12
Draw and clinician-review fee One per order of single tests; not charged when the order includes a Blood Work bundle$29

Typical self-pay total: $89 visit + $41 labs = $130

Your clinician may prefer Diabetes & Kidney panel ($69: a1c, metabolic panel, cholesterol and urine albumin).

Self-pay prices are billed by Passion Health. If you use insurance — including Medicare and Medicaid — the lab (CPL) bills your plan directly for the tests under your plan's lab benefits, and the prices above don't apply. Blood is drawn at a Passion Health clinic in North Texas or at any CPL patient service center in Texas; your clinician reviews your results and contacts you.

Questions

Questions about prediabetes care

How much does an online prediabetes visit cost?

The flat cash rate is $89 for a standard evaluation, or use your insurance and pay your plan's cost-share — we show you an estimate before review starts. Labs and imaging are billed separately. Medication is paid at your pharmacy. If the clinician doesn't accept your case for online care, for any reason, the full fee is refunded to your card.

Will I talk to someone, or is it just a form?

A Texas-licensed Passion Health clinician reviews every intake and decides your plan — not software. For prediabetes, Texas law lets the clinician evaluate you from a condition-specific intake plus your photos and records, and they'll call or video you whenever something needs a conversation. New patients using insurance have a short video visit, because plans pay for a visit, not a form.

Is a prescription guaranteed?

No. You're paying for a licensed clinician's evaluation and plan. Many visits end with a prescription; some end with a different plan, a test, or a referral. Controlled medicines — stimulants, benzodiazepines, opioids, testosterone, sleep aids like zolpidem — are never prescribed at an online visit.

I'm already a Passion Health patient. Can I use this?

If you have insurance, we'll book you with your own Passion Health clinician instead — in person or by video, billed to your plan like any visit, with no online fee. Your clinician knows your history and can adjust your whole plan. If you're an established patient without insurance, continuing a treatment we already prescribed uses the $39 follow-up rate; a new problem uses this page's price.

Where do I get labs drawn, and what do they cost?

If your clinician orders labs, they're priced through Passion Health Blood Work: for prediabetes that's usually Hemoglobin A1c, $41 including the $29 draw-and-review fee, through Passion Health Blood Work (self-pay prices, billed by Passion Health). If you use insurance, including Medicare or Medicaid, the lab (CPL) bills your plan directly and those prices don't apply. You can have blood drawn at a Passion Health clinic in Dallas–Fort Worth or walk into any CPL patient service center in Texas with a photo ID; your clinician reviews your results and contacts you.

How much can lifestyle changes lower my risk?

Quite a lot for many people. In a large US study, the Diabetes Prevention Program, people with prediabetes who lost about 7% of their body weight and were active about 150 minutes a week lowered their chance of developing type 2 diabetes by 58% over about three years. Results vary, so regular A1c checks still matter.

Should I take medicine for prediabetes?

Not everyone needs it, and lifestyle changes come first for most people. Your clinician may discuss a medicine if your risk is higher, for example if your BMI is 35 or more, you're under 60, your A1c keeps rising, or you had diabetes during a pregnancy. Any medicine is a shared decision, made only if appropriate.

Do I need to fast for an A1c test?

No. The A1c reflects your average blood sugar over the past two to three months, so you can eat normally before the draw. If your clinician also orders a fasting glucose or cholesterol panel, you may be asked to fast for at least 8 hours first. Some conditions, such as anemia or certain hemoglobin variants, can make the A1c less reliable.

The Texas law behind this visit

A clinician may form a valid patient relationship online using store-and-forward technology — your condition-specific intake, photos and records — paired with a call or video when needed, as long as the in-person standard of care is met (Tex. Occ. Code §111.005(a)(3), §111.007). Your clinician gives you guidance on follow-up care and, with your consent, sends new patients' visit notes to their primary care physician within 72 hours (§111.005(b)).
