Short answer

Can I get hypothyroidism care online in Texas?

Yes, for adults 18 and older in Texas. Tired, cold, or gaining weight? A clinician reviews your symptoms, thyroid labs and levothyroxine dose, without a waiting room. Review is by a Texas-licensed Passion Health clinician, 24/7 on weekdays and 7 AM–9 PM CT on weekends. The flat price is $89, or use insurance. If labs are needed, your clinician usually orders the Thyroid panel (TSH and free T4), $49, through Passion Health Blood Work, drawn at a Passion Health clinic or any CPL patient service center in Texas; with insurance, the lab bills your plan directly. Medicines a clinician may consider include levothyroxine — never controlled medicines, and a prescription is never guaranteed. If you have a lump in your neck that is new or growing quickly, you need to be seen in person today instead (at our Dallas–Fort Worth clinics, or urgent care near you), and you don't pay the online fee. Already a Passion Health patient with insurance? We book you with your own clinician instead.

About the condition

What to know about underactive thyroid

Hypothyroidism means your thyroid gland makes too little thyroid hormone, so many of your body's processes slow down. In the US, the most common cause is Hashimoto's disease, an autoimmune condition. Others include thyroid surgery, radioactive iodine treatment and some medicines. It is more common in women and after age 60, and it often runs in families.

Is Hashimoto's disease the same as hypothyroidism?

Not exactly. Hashimoto's disease is an autoimmune condition that slowly damages the thyroid; it is the most common cause of hypothyroidism in the US and can cause neck swelling (a goiter). A TPO antibody blood test can support the diagnosis, but not everyone with Hashimoto's needs medicine right away, so your clinician weighs your TSH, free T4 and symptoms together.

Before you start

Is online care right for you?

A good fit for online care

  • You've been diagnosed with hypothyroidism or Hashimoto's and need a levothyroxine refill or dose check
  • You have a TSH result from the last 12 months, or you're willing to have one drawn before your plan is finalized
  • You have symptoms such as tiredness, feeling cold, constipation or dry skin and want your thyroid checked
  • Your thyroid labs from another office were abnormal and you want them reviewed
  • You're planning a pregnancy and want your thyroid dose reviewed ahead of time

Book an in-person visit instead if

  • A lump in your neck that is new or growing quickly Same day
  • A racing or pounding heartbeat since starting or changing your thyroid medicine Same day
  • You're pregnant or just found out (keep taking any thyroid medicine, and tell us or your OB within 1 to 2 days) Within days

Not near one of our clinics? Online Care is open to adults anywhere in Texas; our clinics are in Dallas–Fort Worth. If the safety questions or your clinician show you need to be examined, we tell you what kind of care you need and how soon — an emergency room, an urgent care center, or a clinician near you. If you've already paid, the full fee is refunded and you get a written summary of your visit to take with you.

Call 911 or go to the ER now if you have

Call 911

  • So drowsy you can barely stay awake, or new confusion
  • A very slow pulse (under 50 beats a minute) with dizziness, weakness or fainting
  • Blood sugar under 70 that won't come up with treatment
  • A known adrenal problem with vomiting, very low blood pressure or confusion

Emergency room now

  • Neck swelling with trouble swallowing or breathing
  • Blood sugar over 300 with ketones, vomiting, belly pain or fast breathing
  • A known thyroid problem with fever, racing heart and agitation

Also call 911 for chest pain, trouble breathing, stroke signs, a sudden severe headache, fainting, a seizure, severe bleeding, new confusion, throat swelling, an overdose or thoughts of harming someone else — and call or text 988 for thoughts of harming yourself. If screening routes you before you pay, nothing is charged. If a clinician routes you after you've paid, the full fee is refunded.

Already a Passion Health patient?

If Passion Health is your primary care practice and you have insurance, book this with your own clinician — in person or by video, billed to your plan like any visit, with no online fee. Your clinician knows your history and can adjust your whole plan, not just this one problem. Online underactive thyroid care is for new patients, uninsured patients and patients outside Dallas–Fort Worth; established patients without insurance pay the $39 renewal rate.

How it works

How your underactive thyroid visit works

  1. 1
    Answer the underactive thyroid intake

Upload prior results or home readings if you have them.

  2. 2
A clinician reviews it the same day

If labs would help, the order goes to a Passion Health clinic, or to any CPL patient service center in Texas.

  3. 3
    Results go to your clinician

Your clinician reviews them and contacts you with your plan.

  4. 4
    Follow-up on a schedule

Check-ins and repeat labs timed to your condition; 30 days of messaging.

Your care

What to expect from your clinician

What your clinician may consider

  • Reviewing your symptoms, past TSH and free T4 results, and how you take your medicine (timing, food, coffee and other pills)
  • A TSH and free T4 blood test, and sometimes TPO antibodies, if you don't have recent results
  • Starting levothyroxine only when blood tests confirm an underactive thyroid, with a lower starting dose if you're over 60 or have heart disease; dose changes are based on a TSH from the last 2 months, with a repeat TSH about 6 weeks after any change
  • Checking for things that can affect your results or how well the medicine is absorbed, such as biotin, iron or calcium supplements

Medicines a clinician may consider

Levothyroxine

What these medicines may cost without insurance

Example cash prices for the medicines listed above
Levothyroxine tablets30 tablets$11.16–$11.66

Generic prices are mail-order cash prices listed by Mark Cuban Cost Plus Drug Company on October 15, 2026, including its $5.00 pharmacy fee and $5.25 standard shipping. A shorter course costs less.

Shown so you can plan, not to point you to a pharmacy. Any Texas pharmacy can fill your prescription, and its cash price may be higher or lower. With insurance, your plan sets your cost, and a cash purchase usually doesn't count toward your deductible. If you have Medicare or Medicaid, ask the pharmacy to use your coverage first. Prices change. Passion Health doesn't sell medicine and receives nothing from any pharmacy or drugmaker. A prescription is never guaranteed.

Shown so you know what to expect. The reviewing clinician decides what, if anything, is appropriate for you — a prescription is never guaranteed, and controlled medicines are never prescribed online.

Labs, tests and Passion Health services

  • If your clinician recommends a TSH and free T4, you can have them drawn at a Passion Health clinic or at any CPL patient service center in Texas.
  • If your clinician recommends a thyroid ultrasound for a lump or nodule, they can refer you to an imaging center for it.

How much does an online underactive thyroid visit cost?

Standard evaluation
$89
  • Everything in Quick
  • Lab orders when needed (labs billed separately)
  • 30 days of secure messaging

Using insurance: we're in network with BCBS of Texas, Aetna, UnitedHealthcare, Cigna, Humana and Healthcare Highways, and we verify eligibility for every plan before review — you see your estimated cost-share first. New patients have a short video visit, because plans pay for a visit, not a form. Same-day review applies to self-pay visits; insurance visits begin once we've verified your plan, which is the next business morning if you submit overnight or on a weekend. Medicare, Medicaid, CHIP and TRICARE are billed to the program, and the cash rate isn't offered for covered services.

Full refund if your case isn't accepted: if the clinician doesn't accept your case for online care, for any reason, the full fee is refunded to your card within 10 business days, and we help you get the care you need: a same-day visit at one of our Dallas–Fort Worth clinics or, if you're not near one, clear advice on where to be seen and a written summary of your visit. With insurance, no claim is submitted for the online review. Paying the flat price yourself? You can ask for a written Good Faith Estimate before your visit.

Labs your clinician orders are priced through Passion Health Blood Work if you self-pay, or billed to your plan directly by the lab (CPL) if you use insurance; imaging is billed by the imaging center and medication is paid at your pharmacy. Passion Health doesn't sell, ship or mark up medication.

Follow-up

A 48-hour check-in, 30 days of secure messaging, and lab follow-up when labs were ordered.

Once your dose is stable, refills are usually for 90 days, with a TSH every 6–12 months and about 6 weeks after any dose change. Uninsured patients on a stable dose may continue with a $39 renewal visit.

Labs your clinician may order

Priced through Passion Health Blood Work

TSH and free T4 are the standard check and the dose-adjustment test. Ordered only if your clinician decides it's needed — the visit price never includes a test you don't need.

All Blood Work tests
Thyroid panelTSH and free T4$49

Typical self-pay total: $89 visit + $49 labs = $138

Self-pay prices are billed by Passion Health. If you use insurance — including Medicare and Medicaid — the lab (CPL) bills your plan directly for the tests under your plan's lab benefits, and the prices above don't apply. Blood is drawn at a Passion Health clinic in North Texas or at any CPL patient service center in Texas; your clinician reviews your results and contacts you.

Questions

Questions about underactive thyroid care

How much does an online underactive thyroid visit cost?

The flat cash rate is $89 for a standard evaluation, or use your insurance and pay your plan's cost-share — we show you an estimate before review starts. Labs and imaging are billed separately. Medication is paid at your pharmacy. If the clinician doesn't accept your case for online care, for any reason, the full fee is refunded to your card.

Will I talk to someone, or is it just a form?

A Texas-licensed Passion Health clinician reviews every intake and decides your plan — not software. For underactive thyroid, Texas law lets the clinician evaluate you from a condition-specific intake plus your photos and records, and they'll call or video you whenever something needs a conversation. New patients using insurance have a short video visit, because plans pay for a visit, not a form.

Is a prescription guaranteed?

No. You're paying for a licensed clinician's evaluation and plan. Many visits end with a prescription; some end with a different plan, a test, or a referral. Controlled medicines — stimulants, benzodiazepines, opioids, testosterone, sleep aids like zolpidem — are never prescribed at an online visit.

I'm already a Passion Health patient. Can I use this?

If you have insurance, we'll book you with your own Passion Health clinician instead — in person or by video, billed to your plan like any visit, with no online fee. Your clinician knows your history and can adjust your whole plan. If you're an established patient without insurance, continuing a treatment we already prescribed uses the $39 follow-up rate; a new problem uses this page's price.

Where do I get labs drawn, and what do they cost?

If your clinician orders labs, they're priced through Passion Health Blood Work: for underactive thyroid that's usually the Thyroid panel (TSH and free T4), $49, through Passion Health Blood Work (self-pay prices, billed by Passion Health). If you use insurance, including Medicare or Medicaid, the lab (CPL) bills your plan directly and those prices don't apply. You can have blood drawn at a Passion Health clinic in Dallas–Fort Worth or walk into any CPL patient service center in Texas with a photo ID; your clinician reviews your results and contacts you.

Can I get my levothyroxine refilled online?

In many cases, if you've already been diagnosed and have had a TSH test in the last 12 months. Without a recent TSH, your clinician will usually order one first and decide about a refill once the result is back. Refills aren't automatic: your clinician reviews your dose, symptoms and other medicines. Once you're stable, a TSH every 6 to 12 months is typical.

Why does my clinician ask about biotin, iron and calcium?

Biotin, often sold in hair, skin and nails supplements, can interfere with some lab tests, including thyroid tests, and make results look falsely high or low. If you take it, your clinician may ask you to pause it for a couple of days before your blood draw. Iron and calcium are different: they can block levothyroxine absorption, so they're usually taken at least four hours apart from it.

I already see a Passion Health clinician for my thyroid. Should I use this?

Online thyroid care here is mainly for new patients, people without insurance, and Texans who live outside DFW. If Passion Health is already your primary care practice and you have insurance, the intake will point you to book with your own clinician, in person or by video, billed as a normal visit, with no online fee. Uninsured established patients on a stable dose may be offered a $39 renewal.

Can I start thyroid medicine because of my symptoms?

Not on symptoms alone. Tiredness, feeling cold and weight gain have many causes. Levothyroxine is started only when blood tests confirm an underactive thyroid, usually a high TSH with a low free T4, or a TSH above 10 on repeat testing. A mildly high TSH is usually rechecked in 2 to 3 months before any treatment, and in older adults it often isn't treated. If you're over 60 or have heart disease, your clinician starts with a low dose and raises it slowly. Dose changes are made only on a TSH from the last 2 months.

I'm pregnant and have an underactive thyroid. What should I do?

Keep taking your thyroid medicine, and tell us or your OB within 1 to 2 days. Pregnancy raises how much thyroid hormone you need, so your clinician will usually raise your dose right away (often by about 2 extra doses a week) and check your TSH about every 4 weeks in the first half of pregnancy, working with your OB. If you're pregnant and not on thyroid medicine but have had abnormal thyroid tests or symptoms, tell us too: the intake routes every pregnancy to an in-person visit within a few days.

The Texas law behind this visit

A clinician may form a valid patient relationship online using store-and-forward technology — your condition-specific intake, photos and records — paired with a call or video when needed, as long as the in-person standard of care is met (Tex. Occ. Code §111.005(a)(3), §111.007). Your clinician gives you guidance on follow-up care and, with your consent, sends new patients' visit notes to their primary care physician within 72 hours (§111.005(b)).
