About the condition

What to know about kidney stones

A kidney stone is a hard deposit of minerals and salts that forms inside the kidney. Small stones may pass unnoticed, but a stone moving toward the bladder can cause severe pain in the side or back that spreads to the groin, with blood in the urine, nausea and vomiting. Not drinking enough fluids and diet raise the risk.

What a kidney stone evaluation includes

A same-day visit usually includes a urine test, an exam, pain and nausea treatment, and a decision about imaging, such as a CT scan or ultrasound. If you pass a stone, save it so it can be sent for analysis. After a first stone or repeat stones, your clinician may recommend blood and urine tests to find out why stones are forming and how to prevent the next one.

Before you start

Who should book a same-day visit

Book a same-day visit if

  • Pain in the side or back that comes in waves, with no fever, that over-the-counter pain medicine keeps manageable
  • Blood in your urine with side or groin pain
  • You've had kidney stones before and recognize the pain
  • You've passed a stone and want to prevent the next one

Call 911 or go to the ER now if you have

Emergency room now

  • Fever or chills with side or back pain
  • Vomiting and you can't keep fluids or pain medicine down
  • You can't pee, or you have only one kidney or a kidney transplant
  • You're over 60 and this is the first time you've had pain like this
  • Pain so bad that over-the-counter pain medicine doesn't help
  • Possible HIV exposure in the last 72 hours (PEP medicine must start fast)
  • Sudden pain or swelling in a testicle
  • Sexual assault
  • Pregnant, with pelvic pain or bleeding

Also call 911 for chest pain, trouble breathing, stroke signs, a sudden severe headache, fainting, a seizure, severe bleeding, new confusion, throat swelling, an overdose or thoughts of harming someone else — and call or text 988 for thoughts of harming yourself. There's no charge for the online safety check. If it tells you to call 911 or go to an emergency room, do that instead of booking.

Questions

Questions about kidney stones care

How much does the visit cost?

There's no online fee. Your visit is billed as a normal clinic visit — to your insurance with your usual copay or deductible, or as a self-pay clinic visit. Any tests done that day are billed separately — to your insurance, or at the self-pay price shown to you before the test.

Why can't this be done online?

Kidney stones needs a hands-on exam — and sometimes a test — to be evaluated safely. Rather than charge for an online visit that would end in "please come in", we screen for emergencies and book you straight into a same-day visit at one of our Dallas–Fort Worth clinics. Outside Dallas–Fort Worth, please see an urgent care center or your own clinician.

Why does kidney stone pain need an in-person visit?

Pain in the side or back can come from a stone, but also from a kidney infection, appendicitis, a problem with the main artery in the belly, or other urgent causes. Telling them apart needs an exam, a urine test and sometimes blood tests or imaging the same day. A stone with infection or a blocked kidney can become serious quickly, so it isn't handled online.

How can I prevent another kidney stone?

For most people, the most important step is drinking enough fluid to make about 2 to 2.5 liters of urine a day. Limiting salt and large amounts of animal protein helps too. Depending on your stone type, your clinician may suggest other diet changes, tests to find the cause, or medicine. Don't cut out calcium-rich foods unless advised, because that can backfire.

The Texas law behind this visit

This condition needs a hands-on exam to meet the standard of care, so it is booked in person. The standard of care online is the same as in person, and no agency may impose a higher one (Tex. Occ. Code §111.007; 22 TAC §175.2).
