What is Medicare?

Medicare is a federal health insurance program you become eligible for once you reach the age of 65. People with certain disabilities can qualify for Medicare before they are 65, as can those diagnosed with end-stage renal disease.

The program has three distinct parts.

Part A

This coverage includes hospital stays, skilled nursing facility care, hospice care, and limited home health care benefits. Most people don’t pay a premium for this coverage.

Part B

This coverage includes physician visits, preventive care, certain medical equipment, and outpatient services. Everyone pays a premium for Part B coverage; if you earn more than $97,000 (individual) or $194,000 (couple), you’ll pay a higher amount.

Part D

This coverage includes prescription medications and vaccines. The monthly cost of these plans varies based on which option you select.

Medicare allows you to see the doctor of your choice as long as that practitioner accepts Medicare and is currently accepting new patients. The team at Passion Health Primary Care welcomes Medicare patients.

What is a Medicare wellness visit?

These annual preventive health visits offer a chance to check in with your provider to review your current health and identify areas where improvements can be made.

Wellness visits offer the chance to avoid chronic disease and feel and function your best daily.

  • Medication review
  • Routine health checks
  • Personalized risk assessment and discussion of ways to reduce those risks
  • Review of screening schedule to ensure you get the testing you need
  • Advance care planning

What kinds of services are available through Medicare coverage?

Medicare coverage includes preventive health services like wellness visits and physical exams. If you become sick or injured, acute care visits or “sick visits” include a thorough diagnostic assessment and personalized treatment plan.
